The listener hears thru the microphone in the hearing aid or cochlear implant. Either the loop itself is off, or the t-coil is not active.
Sound reaches the listener from the speakers. Some sound is reflected off of the walls and floor, causing echos. There is also sound from other sources in the room - people, machinery, etc.
